Flu and COVID-19 Vaccine Clinic at the Titan Student Union: Now Open to Families & New Date Available

 


 

Colleagues,

 

As a reminder, the CSUF and Walmart employee on-campus flu and COVID-19 vaccine clinic is this Wednesday, November 3 and Thursday, November 4 from 9 am – 3 pm in the Titan Student Union (TSU), Pavilions B & C.

Due to popular demand we have added a new date on Tuesday, November 9 and vaccines are now available to family members on all dates.

Parking: Guests may park on the first floor of the State College Parking Structure, using the ParkMobile app to pay. Download the ParkMobile app here.

 

Appointments: Click here to sign up for an appointment in advance or scan the QR code with your mobile device camera. Walk-ins will be accepted only on a space-available basis.

 

Vaccine Options: When registering, please note in the comment box what vaccine(s) you would like to receive from the options below. Those who wish to do so may receive both the flu vaccine and one of the COVID-19 vaccines at the same time.

  • Flu vaccine (ages 3 and up)
  • Pfizer COVID-19 booster (ages 18 and up)
  • Pfizer COVID-19 1st dose (ages 12 and up)
  • Pfizer COVID-19 2nd dose (ages 12 and up)
  • Moderna COVID-19 booster (ages 18 and up)
  • Moderna COVID-19 1st dose (ages 18 and up)
  • Moderna COVID-19 2nd dose (ages 18 and up)

 

Participating employees must provide their insurance card and complete all forms required for the vaccine(s) they would like to receive. Those seeking a COVID-19 Pfizer or Moderna booster shot or a 2nd dose of the Pfizer or Moderna COVID-19 vaccine, must also bring their original vaccination card or a photograph of it. If you received the J&J vaccine over 2 months ago, you are eligible for the Pfizer or Moderna booster. In addition, those receiving a COVID-19 vaccination should wait 15 minutes in the clinic area after receiving the vaccine to ensure there are no complications.
