Bereavement Leaves

The CSU Bereavement Leave Policy provides paid time off for the purposes of preparing, attending and/or travelling to a funeral for the employee’s immediate family or close relative.

 

Employee Assistance Program (EAP)

CSUF's employee assistance program is available for faculty, staff and their households’ for individual assistance and support designed to help individuals cope with personal challenges that may adversely affect their lives, behavior, and/or performance. Most services are at no cost.

 

After Death Benefit

Many factors affect death benefits, so it is not possible to know exactly what is payable until CalPERS receives and reviews all of the required documents after a death is reported. Contact CalPERS at (888) 225-7377 or visit Headquarters or Regional OfficesOpens in new window to information on the application process.

 

Beneficiary Designation

Death benefits are paid according to the valid beneficiary designation on file. Benefits may be paid per statutory beneficiary order if there is no designation on file or the designation was revoked. CalPERS will determine who the death benefits are payable to after receipt and review of all the required documents

 

COBRA Benefits

If health benefits are canceled and the beneficiary is not eligible to continue coverage, beneficiaries may be eligible for COBRA.

 

Retirement Designation

Death benefits range from a simple return of contributions (plus interest) to a monthly allowance.

 

Income Tax Designation

For information about federal tax withholding, contact the IRS at (800) 829-1040 or visit irs.gov. For information about state tax withholding, visit the California Franchise Tax Board website at ftb.ca.gov.

Faculty/staff may use this form to update your name, address or tax status/withholding. To update your name, you must also provide your new Social Security card with your new name to Payroll Services, CP-700 who will make a copy in your record.

CALPERS Update

To update your information with CalPERS, such as name or address change, marriage or divorce, or domestic partnership change, visit the CalPERS website to make changes directly on their website. Navigate to the life events section to get started.

Leave Information and Lactation Program

Parental and Pregnancy Disability Leaves

Parental leave (maternity, paternity, adoption) and pregnancy disability leaves are paid leaves of up to 30 days associated with the birth of an employee’s own child or the placement of a child with the employee in connection adoption or foster care.


Lactation Program

Rejuvenation Spaces contain a comfortable sofa or chair, access to electricity, and other amenities to support faculty, staff, students, visitors, and job applications with a private, secure, and sanitary space to use for lactation.
