All employees are required to complete and sign the Acknowledgement of Mandated Reporter Status and Legal Duty to Report Child Abuse and Neglect which functions as a notification to each employee that he/she is a mandated reporter and an acknowledgement that he/she has been notified. The Acknowledgement states that the employee knows his/her legal reporting obligations and will comply with them. The Acknowledgement must be completed, signed and maintained on file.
New employees must complete the Acknowledgement within four weeks of their start date.
Faculty and Unit 11 (TA, GA, or ISA)
Complete the Acknowledgement, print and sign it, retain a copy for
your records, and return the original to your department coordinator.
If you do not have access to a copier, please contact the department staff for

Complete the Acknowledgement, sign it and make a copy for your records, and return the original to Human Resources, CP-700, either in person or through campus mail. If you do not have access to a copier, please contact the department staff for assistance.

Employees must report suspected child abuse or neglect:
- Immediately, or as soon as practically possible: Call the University Police Department at 657-278-2515.
- Within 36 hours of receiving the information concerning the incident: Complete the Suspected Child Abuse Report-SS 8572 and send, or electronically transmit, it to the University Police. Use the Department of Justice Instructions for filling out the Abuse Report.
Note: In case of an emergency or if a crime is in progress, employees should always immediately call campus police or 911.
